Job Summary
We have an exciting opportunity for an experienced, highly motivated practice nurse to join our dynamic nursing team. The successful candidate will use their nursing skills to deliver the highest standards of nursing care in accordance with evidence-based practice and national protocols.
Previous GP practice nursing experience is essential, though additional training can be offered where needed. We encourage the successful candidate to lead on areas of patient care that align with their interests and the practice’s requirements and vision.
Working patterns and hours are negotiable, allowing for flexibility to suit both the practice and the successful candidate’s needs. This is an excellent opportunity for an experienced nurse to develop their career within a supportive and well-regarded practice.
Main Duties of the Job
The successful candidate will practice autonomously without direct supervision and develop an understanding of general practice care delivery standards, including QOF points, and support the practice to achieve these targets.
You will utilize your clinical skills in long-term chronic disease management including asthma, CHD, CKD, diabetes, dementia, hypertension, and respiratory conditions. Additional responsibilities include immunizations, wound care management, ECG recording, and performing diagnostic procedures such as BP, peak flow, and urine testing.
Spirometry recording and interpretation, cervical cytology, contraceptive and sexual health advice, NHS Health checks, health promotion, and travel health services are also key components of this role.
You will ensure the safe storage, rotation, and disposal of drugs and vaccines, maintain good communication with team members regarding clinical findings, and participate in clinical meetings while promoting the protection of children and vulnerable adults in accordance with local safeguarding procedures.
Clinical Responsibilities
As a practice nurse, you will adhere to the Infection and Control policy for the services provided and prescribe medication within the limitations of current extended non-medical prescribing legislation.
You will assess and evaluate outcomes of your practice, service interventions, and treatments, reporting any shortcomings to the Practice Manager. This includes being instrumental in supporting the Infection Prevention Control (IPC) lead by providing audits, training, support, and evidence of compliance.
You will develop, implement, and embed effective care management programmes for frail and housebound patients, assess patient needs to ensure appropriate healthcare provision incorporating evidence-based practice.
Implementing and evaluating individual treatment plans for chronic disease patients that promote health and wellbeing, identifying and managing patients at risk of developing long-term conditions, and providing routine nursing care according to clinical evidence, NICE, and NSF guidelines.
Additional Clinical Services
You will provide wound care including dressings, suture removal, and ulcer care, review patient medications to enhance compliance, and provide guidance on prescribed and over-the-counter medications regarding side effects and dosages.
Coordinating travel medicine services, requesting pathology services, processing results, and overseeing chronic disease clinics are essential aspects of this role. You will maintain accurate clinical records in accordance with extant legislation and ensure SNOMED CT codes are used effectively by all nursing team members.
Developing, implementing, embedding, and participating in well woman and well man clinics, ensuring compliance with the practice chaperone policy, and assisting GPs with minor surgery when required will be part of your responsibilities.

Essential Qualifications
Registered Adult nurse on the NMC register with a practice nurse qualification or accredited long-term condition modules. These qualifications are essential for providing the high-quality care we expect from our nursing team.
The ability to demonstrate a broad and sound knowledge of complex and long-term condition management in primary care is crucial for this role. This knowledge will enable you to provide comprehensive care to our diverse patient population.
Essential Experience
Previous experience in one of the following settings is essential: Primary care/GP practice, Emergency Care Centres, or Out of hours facilities/Walk In Centre. This hands-on experience will ensure you can adapt to the demands of our busy practice.
You must be able to practice autonomously without direct supervision and have experience in diagnosing and managing acute and chronic conditions. The ability to prioritise health issues and intervene appropriately is also essential for this role.
Desirable Experience
Experience of SystmOne electronic patient record system is desirable. Familiarity with this system would help you quickly integrate into our practice’s digital healthcare environment.
Additional desirable experience includes developing and implementing health promotion and wellbeing programmes, coordinating vaccination programmes for adults and children, and supporting patients in the use of prescribed or over-the-counter medicines within your scope of practice.
