As part of the team, you’ll deliver compassionate care through outreach and in-reach support to individuals living in their own homes and extra care housing settings.
The position follows the Active Support Model, focusing on doing things with people rather than for them. This approach promotes choice, independence, and participation at every opportunity. The role involves significant lone working, particularly during outreach visits or when supporting individuals in their own homes. You’ll need to work safely, independently, and confidently while strictly following organisational procedures. Additionally, you’ll provide -1 supervision to Band 3 Support Workers, mentoring and supporting less experienced team members.
Additional Duties
