Job Summary
Area East Continuing Healthcare Care Team has a unique opportunity for an enthusiastic, motivated RMN (Registered Mental Nurse) to join as the Continuing Health Care Lead Nurse OPMH Band 7. This leadership role involves managing a team of Nurse Reviewers who visit patients in care homes and their own homes to assess needs related to NHS Funding, in accordance with National guidance.
Location and Working Conditions
The position is based at Preswylfa, Hendy Road, Mold, CH7 1PZ, North Wales. The role offers a permanent, full-time position with a competitive salary ranging from £48,527 to £55,532 per annum under the Agenda for Change pay scheme. The successful applicant will join the largest health organisation in Wales, providing a full range of primary, community, mental health, acute and elective hospital services for a population of around 700,000.
Key Responsibilities
The role involves extensive coordination and management of Continuing NHS Health Care funding (CHC) in Care Homes and Domiciliary care packages according to the national Framework for CHC and NHS Funded Registered Nursing Care (FNC). The post holder will work closely with Discharge teams across hospital sites, Nursing Homes, Social Services departments, and other NHS professionals.
Leadership and Management
As the Lead Nurse, you will coordinate and scrutinize CHC applications, participate in CHC and FNC reviews, and attend Multi Disciplinary Team (MDT) meetings within the area. You’ll manage the local resolution process, including meeting with families to discuss panel outcomes when individuals challenge eligibility decisions. You’ll also be responsible for resolving Inter Agency Disputes by liaising with Team Managers from Local Authorities.
Collaborative Working
The position requires extensive joint working with Local Authorities to ensure patients in the Independent Sector funded by BCUHB receive high-quality and safe services. You’ll collaborate with CMHTs, DN Services, and Discharge liaison Teams to ensure patients are discharged or transferred in a safe and timely manner. When safeguarding issues are identified, you’ll work with statutory agencies and attend POVA/Strategy meetings.
Training and Development
A key aspect of this role is developing and providing training packages for Multi-Agency staff on CHC issues. You’ll be responsible for promoting good practice across the organisation and supporting the management duties of the CHC team locally. Eligibility and Requirements
Essential for this position is current registration as a Registered Mental Nurse (RMN) with the Nursing and Midwifery Council (NMC). Desirable qualifications include specific knowledge of the CHC Framework and previous managerial experience. Candidates should have a good working knowledge and experience of using the CHC framework.
Professional Registration

Certificate of Sponsorship
