Job Summary
Severnside Medical Practice is pleased to offer a unique opportunity for a Salaried GP to combine clinical practice with a funded Health Equalities Fellowship in a vibrant inner-city training practice. This role is ideal for a GP with an interest in health inequalities, deprivation, and improving outcomes for patients with complex needs. The post offers protected time each week to lead a fellowship project alongside regular clinical sessions within a supportive, multidisciplinary team.
About the Fellowship
There is potential for a permanent role following completion of the fellowship year. This position represents an excellent opportunity for professional development while making a meaningful impact on health equality in a diverse community. The successful candidate will have flexibility to shape a project relevant to both their interests and the needs of the patient population.
Main Duties of the Job
The post will consist of a minimum of four sessions per week for 12 months. This includes one protected Fellowship session per week to undertake a project (ideally focused on frailty and/or multi-morbidity) and three or four normal GP clinical sessions to include on-call duties on a rota.

Job Responsibilities
The post-holder will practice in accordance with accepted good medical practice and the various clinical policies of the Service. This includes one non-clinical session per week for Health Equalities based project work or Personal CPD relating to the Health Equalities project.
Clinical Duties
The post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Professional Responsibilities
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation or external organisations. The candidate will also be responsible for assessing the health needs of patients with undifferentiated and undiagnosed problems.
Health Screening
The successful candidate will be responsible for screening patients for disease risk factors and early signs of illness, as well as screening patients for health education. This proactive approach to healthcare helps improve patient outcomes and prevent health complications.
Care Planning
In consultation with patients and in line with current practice disease management protocols, developing care plans for health-related illnesses. The post-holder will also admit and discharge patients to and from the caseload and refer to other care providers as appropriate.
Documentation and Data Management
Recording clear and contemporaneous consultation notes to agreed standards, collecting data for audit purposes and contributing to the Practice audit programme as required. The candidate will also compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
Prescribing Responsibilities
Prescribing in accordance with the accepted prescribing formulary (or generically) whenever this is clinically appropriate. The post-holder may also take a lead role in some aspects of the clinical work within the team, for example: co-ordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date.
