We are seeking a dedicated Dementia Home Treatment Team Practitioner (DHTTP) to join our multi-disciplinary team at Powys Teaching Health Board. Job location
The position is based at Fan Gorau, Llanfair Road, Newtown, Powys, SY16 2DW. Salary and benefits
We offer a competitive salary ranging from £39,263 to £47,280 per annum, depending on experience and qualifications. This role is permanent and full-time, offering stability and comprehensive benefits package including annual leave, NHS pension scheme, and various wellbeing initiatives.
Key responsibilities
The post holder will be responsible for assessing care needs and developing, implementing, and evaluating programmes of care for clients in the community. You will work flexibly, intensively, creatively, and responsively to clients’ needs, ensuring good links with other professionals and agencies within the designated Dementia, Community Mental Health Team area.
Working hours and pattern
This is a full-time position requiring you to work extended and unsociable hours to meet the needs of service users. You will participate as an active member of the community team, assuming the role of key worker for patients and taking charge of the team caseload in the absence of the Team Leader.
Essential qualifications
To be considered for this role, you must hold a Diploma/Degree in Mental Health Nursing (equivalent to RMN) with current NMC registration, or a Degree level Social care qualification, or a Degree in Occupational Therapy. You should also demonstrate a high level of assessment, risk assessment, planning, and evaluation skills necessary for working with acute mental health crisis.
Essential experience
We are looking for candidates with relevant experience in mental health care and experience of giving and/or receiving supervision. You should have evidence of clinical and professional post-qualification training and development. Knowledge of CTP working practices, Mental Health Act, relevant legislation, and POVA/safeguarding is also essential.
Desirable qualifications
While not essential, additional qualifications and skills such as AOTMH membership, fieldwork educator certificate, Thorn Diploma/Degree, IT skills, Brief Family Therapy qualification, Cognitive Behavioural qualification, Counselling qualification, Clinical Supervision qualification, and venepuncture skills would be advantageous.

Language requirements
The ability to speak Welsh is desirable for this post, but English and/or Welsh speakers are equally welcome to apply. Excellent verbal and written communication skills in English are essential for this role.
