About the Role: Job Summary Responsible for the coordination of placement of Laurelwood patients. This position will work directly with the behavioral therapist and other members of the treatment team to secure placement to ensure continuity of care for patients being discharged from Laurelwood. This position will collaborate daily with the patients and treatment team. Critical to this position are organization skills, customer service, effective communication, resource development and problem solving skills. Relies on extensive experience and judgment to develop and secure placement. Minimum Job Qualifications Licensure or other certifications: Educational Requirements: Bachelors Degree Minimum Experience: 3-5 years experience in a hospital, agency or institution providing related health care services. Experience with patient referral and community resources in service area. Other: Preferred Job Qualifications Preferred Licensure or other certifications: Preferred Educational Requirements: . Works effectively and efficiently in crisis situations. Essential Tasks and Responsibilities Actively engages in discharge planning process, specific to securing placement. Participates in department staff meetings. Also participates in treatment team meetings as needed. Develops and maintains good working relationships with resources in the community. Identifies barriers to discharge and works directly with leadership to develop plan to reduce barriers. Establishes best practice for securing placement to safely discharge patients. Participate in any “discharge planner” meetings scheduled by DBHDD and/or other payors. Develop and maintain resource list. Collaborate with the BH Patient Referral Coordinator and LW Discharge Coordinator as needed. Any other duties as identified by the Therapy Supervisor.
