Saint Louis, Missouri, Additional Information About the Role Barnes-Jewish Hospital has a Community Health Worker opportunity that will advocate, provide support and offer resources to patients living with Sickle Cell Disease. FT, onsite position, Monday-Friday 8a-4:30p. Overview   Barnes-Jewish Hospital is the St. Louis region?s top adult hospital. Preferred Qualifications Role Purpose Community Health Worker provides preventive health improvement services by working closely with interdisciplinary care team, internal/external agencies to address social determinants of health to improve the overall health and well-being of the child and family.  Responsibilities Work daily as a community liaison to identify gaps in care by assisting clients in accessing the necessary resources to remove barriers categorized as social determinants of health. Provide referrals for appropriate services as needed by assisting in assessments/screenings or conducting follow-up care with patients in person, via phone, home visits and/or visits to other settings where clients are accessible. Establish and maintain trusting relationships with clients on a daily basis to effectively work with an inter-disciplinary team, internal departments, and with community organizations. Document and track ongoing activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place. Continuously communicate and expand knowledge of community resources, services and in the hospital or community based agencies the programs provided to reduce cultural and socio-economic barriers between the clients and institutions.
